Paper-Based System

The QoL.CP form is a way to document the resident specific QoL.CP tasks. It should be placed in the appropriate order book for each discipline identified in the QoL.CP form. Facilities may wish to develop their own form; however this template contains all of the key elements. The care plan should:

  • Be written in the voice of the resident,
  • Have a problem statement,
  • List target start date,
  • Identify responsible staff
  • Specify 2-4 individualized care tasks, and 
  • Be kept where the responsible staff has access to them to review and document as appropriate.

Electronic Point of Care System

Kiosk based systems such as CareTracker are easily customizable by the facility to implement the QoL.CP. Each care plan can be broken down into multiple yes/no branching steps to be placed into the system. The QoL care tasks appear in the system as a button whose “light” must be turned out at the end of each shift meaning that the task must be completed or documented as to why the task could not be completed. In this way the care task completion can be document and tracked for compliance in real time.

Voice Activated Documentation

Accunurse is an interactive voice response system for real time point of care charting and communication. The headset voice recognition software records nursing staff responses to pre-determined queries.

Accunurse includes an alert system for “appointments” where the nursing staff member receive a reminder tone at a selected day and time to alert that staff member of an upcoming appointment. This setting is typically used to remind staff to prepare a resident for a doctor’s appointment or an outing but can also be used to remind staff that there is a QoL.CP in place for a resident and what tasks the QoL.CP requires to be completed.
