Overview of Program Materials

 

It takes more than just asking a resident a few questions to understand his/her true quality of life.  Staff need training and practice in how to do the assessment, and the nursing home needs a plan to implement this new system successfully.

In this section of the website we provide the Assessment Materials, an Organizational “Quick-Start” guide, an Assessor “Quick-Start” guide, and examples of care plans.

 

  • Assessment Materials The assessment materials are provided in separate departmental packets for each discipline (Nursing, Social Services, Activities and Dietary).  These packets are ready for use in your facility.  Each packet is comprised of a QoL.DQ (Domain Questionnaire), QoL.ID (In Depth) and QoL.CP (Care Plan) appropriate for each department. Instructions and downloadable copies of each form are available in this section.
  • Organizational Quick-Start To facilitate implementation we’ve made available a “Quick-Start” guide, which gives guidelines on how to assess your organization, create an implementation plan and conduct a Process Review to track your implementation progress.
  • Assessor Quick-Start In the Assessor “Quick-Start” guide, training materials and handouts are available for self-guided study or to train a group of assessors during an in-service. These training materials includes exercises for creating unique care plan tasks, how to ask detailed but sensitive in-depth probes and how to generate the initial care plan tasks for the resident’s quality of life care plan goal.   • Care Plan Examples There are numerous ways to implement care plans into work orders. We have provided several examples of how care plans can be incorporated into a resident’s orders. The care plan examples are presented as a paper form and in formats developed for different electronic record systems.
