The Quality of Life Structured Resident Interview and Care Plan is a system for creating individualized, person-centered care plans in the nursing home.

This interview-based approach to care planning generates the information staff need to tailor a resident’s care plan to their preferences, as well as quantitative measurement of individual and facility-level outcomes. This website presents the program materials, training and implementation guides, and summarizes the findings from the most recent research conducted using the system. The QoL.SRI and QoL.CP as well as training material and implementation guides are available for download after creating a user account.

The QoL.SRI is a multi-dimensional assessment tool for nursing home residents. There are two components to the assessment: the Domain Questionnaire (QoL.SRI.DQ) and the In-depth questions (QoL.SRI.ID). The DQ has closed-ended questions that generate 12 domain scores with established reliability and validity. The ID component has a set of open-ended probes for each of the closed-ended questions. The SRI incorporates decision rules to enable the interviewer to rapidly identify very specific QoL issues to address with individual residents. The residents’ answers to the ID questions are used to generate personalized care plans that are practical and easy to implement in a typical nursing facility. The tight integration between the SRI and the CP assures that the care plan tasks are based on resident preferences and place high priority on enhancing personal autonomy.

The orginal assessment has been divided into separate departmental assessments for implementation purposes and to better fit with the established workflow in nursing homes.  The suggested domain assignments for each department are listed in the table below.
